Alternate Address Additional addresses that are recorded in a student’s record such as for babysitters, alternate pick-up or drop-off addresses, or student summer addresses |
Contact Number Phone number for contact at the alternate address |

Street Name Street name for alternate address. If the LEA is using valid address functionality, the name can be selected from a list of values. If not, the name is entered manually. |
Address Type Description for the type of alternate address, usually alternate pick-up or drop-off for various week days or summer addresses; type is selected from a list of values. |
Out of District Transportation Reason Reason a student is being transported out of district |
Contact Name Name of contact at the alternate address, which may be a relative, babysitter, neighbor, etc. |
Apt # Apartment number for alternate address, if applicable |
Street # Numeric or alpha-numeric postal code assigned to the alternate address |
